Social Worker - Transition Care Service

Amplar Health Home Hospital is expanding its Transition Care Service to Walkerville — providing another welcoming location for public hospital patients who no longer require acute care. Here, patients can rest and recover in comfort, while helping free up hospital beds for those who are acutely unwell. This expansion builds on our innovative model of care, enabling patients to move from metropolitan and peri-urban hospital wards into a supportive sub-acute setting designed for recovery and rehabilitation.
Your Opportunity
We are looking for a full time Social Worker to deliver comprehensive care for to patients with varying complexity on a full-time basis.
Working in an alternate dynamic environment, you will utilise innovative technology and be rewarded in a role where your expertise will contribute to shaping exceptional health outcome for sub-acute patients. Here’s a glimpse of what you will be doing:
- Collaborate with a multidisciplinary team to develop and implement integrated service models and clinical protocols.
- Facilitate group sessions and contribute to health promotion activities as required.
- Provide effective assessment and triage, timely referrals, accurate information, coordinated care and prompt follow up.
- Facilitate discharge planning
- Advocate for patient’s needs and coordinate access to services
- Participate actively in clinical review meetings and continuous professional development.
About you
You are passionate about the provision of innovative models of care and excited by this dynamic way of working. Additionally, you have:
- Degree in Social Work, with eligibility for membership with the Australian Association of Social Workers (AASW).
- Demonstrated experience in a sub-acute or similar healthcare setting.
- Strong communication, advocacy, and problem-solving skills.
- Excellent communication and interpersonal skills, with the ability to adapt to evolving service need.
- Proficiency in using Microsoft Office suite and other relevant documentation systems.
- Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
- If successful, willing and able to provide proof of up-to-date vaccinations.
