As part of our growing Support at Home service, you'll work within a contemporary, clinically-led model that places prevention, early intervention and reablement at the centre of care. Rather than waiting for health challenges to arise, we partner with consumers to proactively maintain their wellbeing, independence, mobility and confidence, enabling them to continue doing the things that matter most.
In this role you'll combine advanced clinical expertise with leadership and coaching responsibilities, supporting consumers with complex health needs while guiding and developing a team of Care Partners. Working alongside allied health professionals, nurses and Amplar Health's broader primary care network, you'll help deliver integrated, person-centred care that empowers consumers to remain connected, capable and thriving in their own homes for longer.

About Role:
As a Senior RN Care Partner, you'll provide advanced clinical oversight and leadership across a portfolio of consumers with varying levels of complexity. You'll support care planning, clinical decision-making and risk management, ensuring care remains safe, effective and aligned to each consumer's goals and preferences.
You will lead and mentor a team of Enrolled Nurse Care Partners, providing clinical guidance, coaching and capability development to support high-quality care delivery. Working through a preventative and reablement-focused lens, you'll help consumers maximise their independence, maintain functional capacity and achieve the best possible health and wellbeing outcomes.
In this role, you'll collaborate closely with consumers, families, healthcare providers and multidisciplinary teams to coordinate care, identify opportunities for early intervention and proactively manage emerging risks. You'll also play a key role in driving clinical excellence, continuous improvement and best-practice care within our innovative digital-first model.
This role is ideal for a Registered Nurse who is passionate about clinical leadership, empowering teams and delivering personalised care that helps older Australians live well at home.
About You:
- Current unrestricted AHPRA Registered Nurse registration
- Strong experience in aged care, community, home care or complex care management
- Proven ability to lead, coach and support clinical teams
- Exceptional clinical judgement and risk assessment skills
- Passion for delivering consumer-focused, preventative and value-based care
