Your Opportunity

We are seeking experienced Paediatric Nurses to join our Care Coordination team based in Adelaide. This role will suit someone who thrives on complexity, experienced in a high acuity paediatric background, communicates with heart, and sees possibilities where others see barriers. You’ll be the steady hand guiding families through some of their most important moments, ensuring every child receives the right care, at the right time, in the right way.

What you’ll be doing;

As a Paediatric Nurse Care Coordinator, you will work closely with children, families, multidisciplinary healthcare providers, and community services to ensure seamless, high-quality care across the healthcare journey.

Key Responsibilities

•    Manage real time escalations from our frontline paediatric nurses in the community
•    Managing consumer care throughout the consumer journey (on-boarding, triaging referrals, initial care planning within scope of practice, care plan review and discharge)
•    Act as a central point of contact for families, clinicians, and external providers
•    Support families in navigating healthcare systems, services, and appointments
•    Monitor patient progress, identify risks, and escalate concerns appropriately
•    Collaborate with multidisciplinary teams to ensure continuity of care
•    Provide education, advocacy, and emotional support to children and their families
•    Maintain accurate clinical documentation and reporting in line with standards

You will be rostered across 7-day working between 9am and 5pm – we offer flexible working options with the ability to work from home.

About you

•    Registered Nurse with current unrestricted AHPRA registration
•    Minimum 5 years’ experience working in paediatrics
•    Strong understanding of care coordination, case management, or complex care (highly desirable)
•    Excellent computer skills and ability to use systems 
•    Excellent communication and relationship-building skills
•    Confident working independently while collaborating within a multidisciplinary team
•    Highly organised with strong clinical judgment and problem-solving skill
