Occupational Therapist Northern & Western Melbourne

We're looking for an Occupational Therapist to join our supportive Allied Health team in a permanent ongoing role (0.6–1.0 FTE) supporting clients across Melbourne's northern and western metropolitan regions. This community-based role offers flexibility, autonomy and the opportunity to make a meaningful difference in the lives of older Australians.

Availability to work Fridays is essential.

About the role

This is primarily a community-based position where you'll support older Australians to maintain their independence, safety and quality of life at home. 

You will provide:

  • Home and environmental assessments 
  • Falls prevention and risk management 
  • Equipment prescription and mobility assessments 
  • Home modification recommendations 
  • Cognitive and functional assessments 
  • Rehabilitation in the home
  • Client-centred care planning and goal setting
  • Collaboration with other health professionals to deliver holistic care 

The majority of your work will be community-based, with a small residential aged care component.

About you

You'll bring:

  • Bachelor of Occupational Therapy (or equivalent) 
  • Current AHPRA registration, or eligibility to obtain registration 
  • Current driver's licence and reliable vehicle
  • Strong communication and relationship-building skills
  • Ability to work independently while remaining connected to a collaborative team 
  • Passion for delivering person-centred care
