The Opportunity

As a Mobile Community Occupational Therapist, you'll make a real difference by helping people live safely, independently and confidently in their own homes. You'll partner with clients to identify practical solutions that enhance their independence, safety and quality of life through home and functional assessments, equipment and home modification recommendations, falls prevention strategies and personalised care planning.

As Amplar Health's Support at Home service continues to grow, you'll be part of an innovative, clinically-led model focused on prevention, early intervention and reablement. Working alongside Clinical Care Partners, nurses and allied health professionals, you'll deliver coordinated care that helps older Australians remain independent at home while contributing to the future growth and development of the service.

With flexible full-time and part-time opportunities available, this role is ideal for both experienced Occupational Therapists and new graduates looking to grow their careers.

About You

· Current unrestricted AHPRA registration as an Occupational Therapist

· Ability to work autonomously and collaboratively within multidisciplinary teams

· Excellent organisational and time management skills

· Current driver licence, access to own vehicle and willingness to travel
