What you’ll do 

• Lead and develop a team of Care Coordinators to deliver safe, high-quality care at scale
• Drive performance, meet operational and financial targets, and optimise workflows
• Champion continuous improvement and innovation across programs
• Build strong relationships across Amplar Health and beyond to grow referral pathways
• Support transformation initiatives from concept through to business-as-usual
• Ensure clinical governance, compliance, and best practice standards are always met 

What you need 

• A clinical qualification with current registration (AHPRA or equivalent)
• Proven experience leading frontline teams (ideally geographically dispersed)
• Strong communication, organisational, and stakeholder engagement skills
• An understanding of care coordination, chronic disease management, and behaviour change
