Amplar Health Home Hospital is expanding its Transition Care Service to Walkerville — providing another welcoming location for public hospital patients who no longer require acute care. Here, patients can rest and recover in comfort, while helping free up hospital beds for those who are acutely unwell. This expansion builds on our innovative model of care, enabling patients to move from metropolitan and peri-urban hospital wards into a supportive sub-acute setting designed for recovery and rehabilitation.

No two days will be the same: you’ll care for patients recovering from surgery, illness, or injury, helping them regain strength and confidence, in this sub-acute nursing role.

 

We are seeking ENs to join the team on a full time or part time (0.6 FTE as a minimum) basis.

  • Monitor patients progress and report changes to a broader multidisciplinary team 
  • Maintain up to date documentation 
  • Contribute to care planning and patient assessments 
  • Support activities of daily living and administer medications 

 

About you 

You’ll enjoy a balanced 7-day rotating roster (mornings, afternoon and nights) offering variety, flexibility, and work-life balance, within a supportive team environment.

  • Experience as an Enrolled Nurse  
  • Current unrestricted AHPRA registration  
  • Certification to dispense medication 
  • Experience in wound care management 
  • Hold a current CPR certificate
  • High attention to detail with a patient focused approach 
  • Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain)
  • If successful, willing and able to provide proof of up-to-date vaccinations
  • Nurses with agency experience in acute care are encouraged to apply
