Amplar Health Home Hospital (AHHH) is expanding its multidisciplinary team, and we are seeking a highly skilled Clinical Pharmacist to play a pivotal role in delivering safe, effective, and patient‑centred medication management across all our programs.

 

You will provide comprehensive, high‑level pharmacy services that optimise medication safety, efficacy, and cost‑effectiveness. You will exercise independent professional judgment, offer therapeutic advice to both patients and clinicians, and ensure all medication practices align with standards and organisational policies.

  • Deliver expert clinical pharmacy services, ensuring safe and effective medication management.
  • Provide evidence‑based therapeutic advice to patients, carers, and clinicians.
  • Apply independent professional judgment in medication review, reconciliation, and optimisation.
  • Support continuity of care across transitions, including admission, discharge, and handover processes.
  • Contribute to medication governance, policy development, and service improvement initiatives.
  • Participate in research, quality improvement, and evidence‑based practice activities.
  • Engage in consumer education to promote safe and informed medication use.
  • Ensure compliance with all standards and organisational policies.


About you 

  • Registered Pharmacist with current unrestricted AHPRA registration.
  • Experience in hospital pharmacy or sub‑acute setting preferred.
  • Detailed orientated with a high attention to detail.
  • Strong clinical knowledge and ability to apply evidence‑based practice.
  • Excellent communication and teamwork skills.
  • Commitment to patient safety, quality improvement, and continuous learning.
  • Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain). 
  • If successful, willing and able to provide proof of up-to-date vaccinations.
