Clinical Care Coordinator

We’re seeking a Registered or Enrolled Nurse to join our Support at Home (S@H) Clinical Pathways team. In this role, you’ll combine clinical expertise with care coordination skills to deliver safe, timely, and client-centred services for older Australians.

What you’ll do:

  • Screen and assess referrals from aged care providers.
  • Allocate clients to the right clinical pathway and coordinate service delivery.
  • Monitor progress, review outcomes, and ensure compliance with Aged Care Quality Standards.
  • Contribute to process design and continuous improvement as the program evolves.
  • Act as a trusted clinical liaison for providers, clients, and families.

What we’re looking for:

  • Current AHPRA registration (RN or EN).
  • Minimum 3 years’ experience in aged care, community care, or rehabilitation.
  • Strong clinical assessment and care coordination skills.
  • Digital proficiency and ability to work virtually.
  • Empathetic, proactive, and client-focused approach.
