Your Opportunity

Tired of working on the frontline? Looking to join a team coordinating care and services to support patients in their own home? We have a part time opportunity 0.6 FTE available to work within the Aged Care program.

Using your clinical skills, you will ensure a comprehensive client centred management plan is achieved for our clients together with:

•    Conducting clients initial phone assessment, care planning and goal setting
•    Managing real-time escalations from our on-road clinicians 
•    Coordination of in-home services to support clients receiving care at home
•    Building and maintaining effective relationships with a variety of different stakeholders including primary and secondary care organisations and your non-clinical support team
•    Monitoring patient progress and updating care plans as appropriate
•    Liaison with healthcare professionals and specialists
•    Home visits to undertake assessments when required 
•    Discharge planning including referrals to long term services
•    Provision of telephone support, education, and advice
 
This role is based in Adelaide, the successful candidate will be offered family friendly hours working a Monday to Friday roster. We are seeking someone to work every Wednesday, Friday and another day of your choice. Best of all this role can be offered as work from home.

You will be consumer, and goal focused whose enthusiasm for this area of work is driven to engage with and empower our consumers to achieve positive outcomes together with:

•    Recent nursing experience supporting consumers within the community setting
•    Experience working within a multi-disciplinary team & working remotely
•    Experience supporting consumers within the community and or aged care setting
•    Experience within the home care package setting is highly desirable
•    Exceptional communication and customer services skills and the ability to build a rapport with consumers and external agencies
•    Ability to work autonomously but contribute to a supportive and engaging team
•    Strong administration skills and the ability to ‘think outside the square’
