As a Care Coordinator – Home Health, you'll play a vital role in coordinating patient care, educating consumers, monitoring treatment plans, and collaborating with healthcare providers to ensure the best possible outcomes. Working alongside a supportive clinical team, you'll use your clinical expertise, problem-solving skills, and compassionate approach to deliver high-quality care that truly matters.

What we're looking for:

Current unrestricted AHPRA registration as a Registered Nurse
Minimum 2 years' clinical experience
Strong communication, stakeholder management, and customer service skills
Confidence using technology and clinical systems
A proactive, patient-focused approach to care
