Amplar Health Home Hospital has expanded its Transition Care Service with a location in Walkerville and Adelaide, providing another welcoming location for public hospital patients who no longer require acute care. Here, patients can rest and recover in comfort, while helping free up hospital beds for those who are acutely unwell. This expansion builds on our innovative model of care, enabling patients to move from metropolitan and peri-urban hospital wards into a supportive sub-acute setting designed for recovery and rehabilitation.

 
Your opportunity 

You will join a team that is bridging the gap between hospital and home. Working closely with clinicians in a supervised environment, you will:  

  • Delivery exceptional patient care  
  • Follow individualised care plan and work as a part of a multidisciplinary team 
  • Support patients with activities of daily living 
  • Observe and report on patient changes  

 
About you 

You will be available to work part time (0.6 FTE minimum), on a 7-day rotating rostering across morning, afternoon and night shift, passionate about patient centred care and excited to be a part of this innovative model of care, in addition to: 

  • Assistant in nursing qualifications – Certificate III in Health Services Assistance, Individual Support or similar relevant qualification 
  • Ideally have recent experience in a similar role 
  • Being a team player, with excellent communication and problem-solving skills 
  • Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain). 
  • If successful, willing and able to provide proof of up-to-date vaccinations.
