The Opportunity

We are looking for a part time or full time (0.6-1.0 FTE) Cardiac Rehab Care Coordinators to join the team supporting a national program, Heart Health at Home.

This opportunity will be an initial fixed term contract for 12 months with the possibility of extension.  

As this is a national role, you can be located anywhere in Australia, and work flexibly, including work from home. 


As a Care Coordinator with significant experience in the area of Cardiac Rehabilitation you will work with clients who have recently been discharged from hospital following a cardiac event. You will utilise a coordinated and integrated approach, facilitating health behaviour change and improving health outcomes through the delivery of health coaching and an innovative remote cardiac rehab program.

With a focus on Cardiac Rehabilitation, you will be:

  • Initiating contact with potential participants and completing an enrolment process;
  • Managing a caseload of participants and coordinating appropriate cardiac rehab plans;
  • Delivering evidence based cardiac rehab education, coaching and support to participants via telephone;
  • Monitoring participants’ health to ensure any risk factors or deterioration are identified and managed appropriately
  • Assisting participants to access and utilise the Cardihab app to support their cardiac rehab program
  • Actively engaging with participants’ GPs and/or cardiologists, to ensure that they are aware of the program and participant’s progress


You will be supported by a Clinical Lead, Medical Advisor and consultant Cardiac Specialist to ensure the service provided is of the highest quality, and underpinned by the latest evidence base.


Please note this role is a non-hands on, clinical role.


About You

You have a natural ability to build rapport with a wide range of clients and healthcare professionals and effectively influence for success. Working in this highly autonomous position you will have an organised and flexible mindset that allows you to meet the unique needs of your clients. Additionally, you will also have the following skills/experience:

  • At least 5 years professional clinical experience as either a Registered Nurse, Physiotherapist or Exercise Physiologist with current registration with AHPRA or ESSA
  • 2+ years’ experience delivering cardiac rehabilitation (phase 2) within the past 5 years;
  • Previous health coaching experience and understanding of health behaviour change methodology
  • Experience in acute and primary health care and care planning
  • Solid understanding of cardiac rehab programs, cardiac disease and evidence-based practice

You will also be working from home so will need to have a suitable office area from which to work and reliable internet connection. From time to time you may also be required to attend team meetings, conferences or other events.
