The Opportunity

We’re looking for a practical, hands‑on Change Manager – Primary Care to support the delivery of change across Amplar Health’s Primary Care portfolio.

This role is ideal for someone who enjoys being close to the work—drafting communications, engaging teams on the ground, and helping people understand and embrace change in practical ways

 

You’ll support transformation initiatives across acquisitions integration, functional and technology change, and clinical improvements. A core part of this role is bringing people along the primary care transformation journey—helping GPs, practice teams and support staff connect with the purpose of change, not just the process. This is a delivery‑focused role for someone who enjoys building trust, capturing hearts and minds, and turning change plans into action.

 

 

Your Impact

You’ll help change land well in busy primary care environments by making it clear, relevant and achievable. Through a people‑centred and practical approach, you’ll support engagement, confidence and adoption—ensuring improvements are embedded into everyday practice.

 

What You’ll Do

  • Support the delivery of change activities across primary care transformation initiatives.
  • Work directly with GPs, clinic teams and support and corporate office staff to understand impacts and day‑to‑day realities.
  • Support change across clinical and corporate initiatives including but not limited to new service delivery models, cost optimisation initiatives, corporate and/or clinical system changes, and policy or compliance updates.
  • Create clear, practical change materials including communications, FAQs, posters, presentations and local engagement activities.
  • Help teams connect with the “why” of change and feel supported through transition.
  • Complete change impact and readiness assessments across people, process and technology.
  • Listen for resistance or uncertainty and respond in practical, people‑focused ways.
  • Track engagement and adoption, and support simple reporting on how change is landing.

 

 

About You

You’re a hands‑on change practitioner who genuinely enjoys working with people and helping them navigate change. You balance structure with warmth, and you’re just as comfortable creating a comms pack or running a clinic briefing as you are completing an impact assessment.

 

You’ll likely bring:

  • Experience supporting or delivering change in healthcare, service delivery or similar environments.
  • Practical experience using change approaches such as PROSCI, ADKAR or Kotter (formal certification not required).
  • Strong written and verbal communication skills, with a natural ability to connect with diverse audiences.
  • Confidence engaging with GPs, practice teams and corporate stakeholders alike.
  • A down‑to‑earth, proactive and adaptable working style.
  • Tertiary qualifications in a business‑related discipline, or equivalent practical experience.

 

 

Experience working in primary care, M&A integrations or environments undergoing rapid change will be highly regarded.
