About this opportunity

As part of our commitment to advancing patient care, we are seeking dedicated Medical Practitioners to play a pivotal role in ensuring the highest standards of medical management for our patients. In this newly created role, you will provide expert medical advice and support to patients; Amplar Health teams; and health practitioners, involved in the management of acute and subacute care in the home.

This role will see you participate in daily huddles with the case coordination team to review incoming referrals, discuss clinical escalations, identify patients requiring medical review, and support discharge planning. In addition, you will:

•    Conduct virtual consultations with patients as needed.
•    Review and approve care plans for high-risk patients based on agreed criteria
•    Manage medications, including initial charting, updates, and limited prescribing during the episode of care
•    Request pathology and radiology where required, review results, and collaborate with treating specialists
•    Liaise with treating specialists and regular GPs for escalations, concerns, and discharge planning
•    Contribute to the development of clinical guidelines, review clinical outcomes at a cohort level, and provide input into models of care

You will be provided with the equipment and technology to enable you to work from home– so this is a great flexible option to extend your career.
We are offering 12-month fixed term positions and you will need to be available for at least 2 sessions a week, which are 3-4 hours in length each. Sessions are available Monday to Friday between 9am to 6pm and weekend mornings only. 

About You
 
With full unconditional AHPRA registration this role suits early career General Practitioners or recently trained hospital residents/registrars, preferably PGY4+ with BPT, FRACGP or FACRRM.

Your ability to apply theoretical knowledge and practical experience in determining the appropriate clinical diagnosis, investigations, and treatment of patients with acute conditions in line with clinical risk and compliance frameworks will be highly regarded. Confidence in providing virtual care will be highly regarded.
