Your Opportunity

As a Registered Nurse Clinical Coordinator you will be responsible for supporting our Transition Care Program and Home Care Package consumers in South Australia to maximise their independence and wellbeing.

This amazing position will see you visit consumers homes in the community to undertake holistic conversations and assessments with consumers and their representatives to create comprehensive care plans with them that support their reablement goals and care requirements.

This role will also assist with case coordination of appropriate TCP consumers:

  • Maximise their ability to achieve their program goals
  • Coordinate complex and high acuity person centre care to improve consumers physical and or cognitive functioning
  • Liaise with external and internal providers as required for service transition
  • Participate in regular Muti-D meetings with internal and external stakeholders
  • Liaise with the relevant TCP metro LHN teams to ensure coordinated service delivery

Your organised and flexible mindset allows you to meet the unique needs of our consumers to maximise their wellbeing. You will be consumer and goal focused whose enthusiasm for this area of work is driven to engage with and empower our consumers to achieve positive outcomes together with:

  • Recent nursing experience supporting consumers within the community setting
  • Experience working within a multi-disciplinary team
  • Experience supporting consumers within the community setting
  • Experience within the home care package/ transitional care package setting is highly desirable
    Exceptional communication and customer services skills and the ability to build a rapport with consumers and external agencies
  • Ability to work autonomously but contribute to a supportive and engaging team
  • Strong administration skills and the ability to ‘think outside the square’

 

Willingness to use your own car to travel throughout Metro and South Adelaide is required with kilometer reimbursement fortnightly.

This role is Permanent Part Time, set at a 0.6FTE over the Monday - Friday work week, with potential for growth. We promote flexible working arrangement in our team and support work from home for part of the working week.
