We have an exciting opportunity for a Cardiac Rehab Care Coordinator to join the team supporting our national program, Heart Health at Home.
As a Care Coordinator with significant experience in Cardiac Rehabilitation you will work with clients who have recently been discharged from hospital following a cardiac event. You will utilise a coordinated and integrated approach, facilitating health behaviour change and improving health outcomes through the delivery of health coaching and an innovative remote cardiac rehab program.
This is a permanent part time opportunity (0.8 FTE) and Work from Home role which require you to have fast, reliable internet and appropriate workspace to ensure privacy whilst working. We will provide you with the equipment, training and support you need to succeed in your role and to ensure the service provided is of the highest quality and underpinned by the latest evidence base.
Please note this role is a non-hands on, clinical role.
About You
You have a natural ability to build rapport with a wide range of clients and healthcare professionals and effectively influence for success. Working in this highly autonomous position you will have an organised and flexible mindset that allows you to meet the unique needs of your clients. Additionally, you will also have the following skills/experience:
- At least 5 years professional clinical experience as either a Registered Nurse or Physiotherapist with current registration with AHPRA
- 2+ years’ experience delivering cardiac rehabilitation (phase 2) within the past 5 years.
- Previous health coaching experience and understanding of health behaviour change methodology.
- Experience in acute and primary health care and care planning.
- Solid understanding of cardiac rehab programs, cardiac disease, and evidence-based practice.
