Your Opportunity

We are seeking an experienced and patient-focused Medical Practitioner to join our Transition Care Service, providing sub-acute care across our Adelaide and Walkerville sites.

 

Working within a supportive multidisciplinary team, you will play a key role in delivering high-quality, person-centred care. You will collaborate closely with nursing, allied health and care teams to support positive health outcomes and enhance patient wellbeing.

About You

You are passionate about delivering outstanding patient care and are excited by the opportunity to work within innovative and evolving healthcare models.

  • Have full unrestricted AHPRA registration with a minimum of five years' post-registration clinical experience.
  • Post graduate medical specialist quals will be viewed highly favourably
  • The ability to work confidently and effectively within a sub-acute setting (bridging the gap between primary and acute healthcare)
  • Strong communication and collaboration skills, with a commitment to multidisciplinary care
  • Hold a valid South Australian 'Working with Children Check' and a National Police Clearance or are willing and eligible to obtain.
  • If successful, willing and able to provide proof of vaccinations.
