The opportunity 

As a Placement Support Officer, you’ll provide essential operational and general support to the Social Work team, helping patients transition from hospital care into the most appropriate residential aged care or community placement options. Your work directly contributes to safe, timely and person‑centred discharge outcomes, ensuring every patient’s next step is coordinated with care and professionalism.

 

You’ll work under the direction of the Social Work Lead in this part-time (0.8 FTE) role, within Transition Care Services (TCS) located in Adelaide’s CBD and Walkerville.

 

You will:

  • Identify, coordinate and facilitate residential aged care and community placement options for patients preparing to leave the transition care service.
  • Build and maintain strong working relationships with aged care providers to support smooth placement pathways.
  • Contribute to safe, timely discharge outcomes by supporting Social Workers with essential administrative and operational tasks.
  • Maintain accurate documentation, track placement progress and support efficient coordination of all placement processes.
  • Ensure all actions align with organisational policies, standards and best‑practice guidelines.

 

About you 

  • Qualification or experience in health administration, community services, aged care, or allied health assistance
  • Demonstrated experience in healthcare, aged care, or community services environment.
  • Sound understanding of aged care pathways, eligibility, and placement processes.
  • Effective communication and interpersonal skills.
  • Attention to detail, problem solving skills and outstanding interpersonal skills.
  • Current driver’s licence.
  • If successful, willing and able to provide proof of up-to-date vaccinations.  
  • Holding a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
