About this opportunity
As the Medical Team Lead for Amplar Health Home Hospital Transition Care Services (TCS), you will play a pivotal role in leading teams to deliver safe, high‑quality care to patients within the Transition Care Service. This leadership position is central to supporting our medical staff, strengthening clinical governance, elevating patient outcomes, and fostering a culture of continuous improvement across multidisciplinary medical teams.
Working as part of the leadership team, you will champion patient safety, guide clinical excellence, and support medical staff to deliver effective, patient‑centred care. Join us in a flexible 0.8 FTE role, working onsite between TCS locations, currently at Walkerville and Adelaide’s CBD.
Key Responsibilities
- Lead and mentor Medical Officers within the TCS, ensuring consistent delivery of safe, timely, and patient‑centred care.
- Embed strong clinical governance frameworks that uphold quality, safety, and accountability across all medical practices.
- Drive a culture of continuous improvement, using data, feedback, and evidence‑based practice to enhance patient outcomes.
- Work closely with a multidisciplinary and leadership teams to coordinate seamless, integrated care.
- Utilise your general practice or relevant other medical specialty to support complex decision‑making and ensure high‑quality care delivery.
- Contribute to the evolution of healthcare delivery models across South Australia, helping shape the future of home‑based and transitional care.
About you
You are a clinician who thrives in leadership roles, values patient‑centred care, and is passionate about improving clinician experience and wellbeing, systems approaches and outcomes. You bring strong communication skills, sound clinical experience and judgement, and a collaborative mindset in addition to:
- Bachelor of Medicine, Bachelor of Surgery or Doctor of Medicine
- Relevant specialist qualifications such as FRACP, FRACGP, FACCRM or experience with additional training to be considered.
- Having full unrestricted AHPRA registration with at least 10 years’ minimum clinical experience
- Demonstrated leadership experience in clinical governance, quality improvement and/or patient safety initiatives
- Having a high level of professionalism, with the ability to relate well with people of all interests, background and abilities and a commitment to delivering exemplary care to our patients
- Passion about equity of outcomes and care of vulnerable people
- Holding a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
