About this opportunity
As a Care Integration Nurse or Paramedic, you will collaborate as part of a broader team working closely with Medical Officers, Nurse Care Coordinators, our service provider network and the Local Health Networks (LHNs).

You will be responsible for delivering patient-centred care, to suitable patients for Amplar Health Home Hospital (AHHH) and undertaking clinical reviews as part of a multi-disciplinary team.

Our design of the AHHH model has been informed by core principles that embody the change that we want to see (and create) in acute care.

This role will involve undertaking visits in the community to our patients’ homes delivering nursing care as required by their care plan. You will have use of our company vehicle for patient visits starting your shift from our office in Eastwood.

This position will be offered as either casual or a part time position. You will be rostered across a 7-day roster across two shifts 7am to 3pm or 2pm to 10pm.

You will be naturally empathetic, observant, attentive, trustworthy, clinically sound and committed to making AHHH a centre of excellence.  

Working in a dynamic environment applying your clinical skills, you will think on your feet and problem solve to ensure our consumers are provided with best practice care. 

You will also have:

  • Bachelor of Nursing or Paramedicine & current unrestricted AHPRA registration.
  • Current Basic Life Support certification.
  • Clinical experience in the management of acute medical and surgical patients, Intravenous cannulation skills, or the willingness to achieve this competency, and experience with comprehensive clinical assessments will be highly regarded.
  • Understanding of National Safety and Quality Health Service Standards.
  • Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain). 
  • If successful, willing and able to provide proof of up-to-date vaccinations.
