Your opportunity 

Join this busy service, delivering patient centred virtual care to acute patients.

As a Care Coordinator, you will join a team making care more accessible. You will establish, monitor and coordinate patients' treatment plans, utilising virtual technology, in partnership with our home visiting provider team.

Use your clinical, people and communication skills to manage exceptional patient care, enhancing and promoting quality of life.

 

About you 

You can work across a 7-day rotating roster between the hours of 7am and 10pm, based in our Virtual Care Centre (VCC) in Eastwood, on either a full time or part time (minimum 0.6 FTE) fixed term contract. You will be excited to apply your clinical skills in a dynamic work environment, in addition to:

  • Bringing significant experience as a Registered Nurse
  • Current unrestricted AHPRA registration.
  • Recent experience in acute care management in a hospital or community setting amongst a multidisciplinary team will be highly regarded.
  • Being able to think on your feet and problem solve.
  • Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.
  • Strong clinical reasoning and assessment skills.
  • Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain). 
  • If successful, willing and able to provide proof of up-to-date vaccinations.
