Our Story
Home Support Services (HSS) is part of the Medibank enterprise and is the leading provider of home hospital care for a range of public and private funders. We provide healthcare and wellbeing services nationally in alternative settings to hospitals, supporting people across the care continuum. We are now looking for a Service Delivery Lead to join our Rehabilitation in the Home program on a permanent basis.
Your Opportunity
As the Service Delivery Lead (or Program Lead) you will be responsible for driving the performance of the site-based Rehab in the Home (RITH) Case Coordinator Team. Reporting to the National Service Delivery Manager and working closely with the other program leaders.
This role is a key leadership post with a mandate to manage operations and people leadership across the RITH Program with a view to delivering exceptional patient centred services. As the Service Delivery Lead, you will:
- Provide supportive leadership and direction to your team, including coaching support, regular informal and formal performance feedback and reviews, and support for relevant training and development
- Maintain program outcomes within practice guidelines and funding requirements
- Provide support and advice to Case Coordinators regarding the management and coordination of patients
- Ensure the program deliverables are met with a skilled workforce who are committed to providing quality care and services
- Actively monitor & review Quality assurance and Clinical compliance, including but not limited to; knowledge of program, attendance, clinical non-compliance, clinical and systemic risks
- Effectively establish and maintain professional relationships with various stakeholders
What will set you apart from the rest?
In order to be a successful applicant for the Service Delivery Lead role, alongside outstanding communication skills and passion for people you will:
- Have a minimum 5 years’ experience as a qualified AHRPA registered health professional
- Have demonstrated effective leadership, communication (written and verbal) and interpersonal communication skills
- Experience in effectively managing staff, including demonstrated ability to lead, motivate and engage staff across diverse work groups
- High level clinical and operational experience within a complex health environment
- Demonstrate effective organisational skills and the ability to meet competing priorities to achieve program deliverables on time and within budget
- Demonstrate ability to achieve organisational change and program outcomes through effective leadership, internal and external stakeholder engagement, planning, problem solving, negotiation and change management
