General Practitioners/Medical Officer - Transition Care Service

Your opportunity   

We are seeking a General Practitioner/Medical Officer for the sub-acute service located onsite in Adelaide. You will work within a multidisciplinary environment where collaboration and patient centred care are at the forefront of everything we do.

This is a unique opportunity to be part of a forward-thinking healthcare model in a supportive and collaborative environment.

This role will work across 7-day roster, day shift only, 8am to 4pm on a part time basis. 

About You
Passionate about patients and keen to explore opportunities in innovative models of care, as well as

• Being fully registered with AHPRA with a minimum 3 years’ experience.
• You are comfortable working in a sub-acute setting 
• Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
• If successful, willing and able to provide proof of vaccinations.
