With the help of a dedicated and highly skilled team, we aim to deliver a higher proportion of acute hospital care in the comfort of patient’s homes providing them greater comfort, experience and choice.

 

Your Opportunity:

 

As a member of our Case Coordinator team, you will apply your clinical skills to ensure comprehensive, seamless care management of clients admitted to our servi. Daily operations include:

 

  • Patient initial phone assessment, care planning and goal setting
  • Coordination of in-home services
  • Monitoring patient progress and updating care plans as appropriate
  • Liaison with healthcare professionals and specialists
  • Discharge planning including referrals to long term services
  • Provision of telephone support, education and advice

You will be knowledgeable about health care practice’s and are surrounded by likeminded team members who live and breathe a service centric culture by providing an impeccable experience to consumers and stakeholders together with:

 

  • Minimum 2 years' experience as a Registered Nurse with an unrestricted AHPRA registration
  • Strong clinical reasoning skills
  • Knowledge and experience (preferred) in:
    • Wound Care
    • PICC line management
    • IV Antibiotics
  • Ability to work in a fast-paced environment
  • Proficient computer skills to enable fast and accurate data entry into patient electronic medical records.

 

Please note we operate 7 days a week and applicants may be required to work a variety of shifts based on a fortnightly roster.
