Working across the CareComplete suite of programs, and in partnership with general practitioners, you will deliver a range of integrated services to provide an additional, coordinated support system to patients with complex health needs.

Due to program expansion, we are looking for a clinical or allied health professional to be the face of CareComplete in Joondalup, WA.
As a Care Coordinator, you will utilise a coordinated and integrated approach to healthcare, facilitating health behaviour change and care coordination with patients across a range of settings, including within primary care practice, in patient homes and virtually via telephone. Key responsibilities in the role include, but are not limited to:
- Embedding health behaviour change principles into the delivery of the program to empower and increase patient ownership of their health.
- Ensuring clinical quality and patient centred care is being provided to program participants, particularly those that help minimise preventable hospital admissions.
- Collaborating with non-clinical support teams in a team care approach.
- Developing effective relationships with primary and secondary care organisations.
To be successful in this role you will be an APHRA Registered Nurse or similarly registered clinical or allied health professional with experience in care coordination. Exposure to the primary, community and/or aged care settings and a solid understanding of chronic disease management principles is also required. Experience in using IT systems for recording and monitoring of patient activity, as well as, an ability to effectively manage and prioritise your work duties, will be key drivers of your success. Most importantly, you will be passionate about influencing positive change in healthcare.
This is a full time role with the requirement to travel within and outside the region. A valid driver's license and personal vehicle are mandatory requirements for the role (travel reimbursements will apply).
