We are looking for a Clinical Team Leader to manage a team of clinical and non-clinical staff (30 hours/week) in the delivery of CareComplete, every day you will enhance, improve and impact the health outcomes of program participants. Through inspiring leadership, the Clinical Team Leader will develop their team to achieve improved patient health outcomes, an enhanced patient experience and reduction in overall cost outlays.
Key responsibilities will include:
- Driving regional achievement of CareComplete objectives;
- Facilitating a team approach to the coordination of all CareComplete, ensuring clarity of objectives and responsibilities for each member of the team;
- Providing inputs into design and refining the optimal workflow across the teams to ensure the most effective clinical and operational delivery models; and
- Fostering positive working relationships with internal and external staff at all levels, and assist in the development and success of team members.
To be successful in this role, you will be an allied health professional or Div. 1 Registered Nurse with a passion for leading teams and assisting people who will benefit from integrated care planning.
You will have experience in working in health programs focused on achieving multiple objectives and in primary, acute, aged care and other community care environments. This includes ideally having existing networks of relationships across a region or regions. You have a relevant professional body registration and strong leadership and management experience, as well as the ability to manage staff working across multiple locations and delivering via multiple modalities.
