Reporting directly to the Clinical Lead you will:
- Coach patients/caregivers to assert a more active role during care transitions
- Assist them in developing long-lasting self-management skills
- Provide on-going support for patients with complex needs via a range of methods including face to face and telephonic support
- Help minimise preventable hospital admissions
For a coach to be compliant with broader health cover legislation, they need to be an experienced allied health professional who is able to claim Medicare rebates – so an Occupational Therapist, Social Worker, Dietician, Physio or Exercise Physiologist. But it’s not credentials that make a coach, more important is the ability to be a facilitator, to model and transfer the skills that patients and their families need so they can solve problems that arise during care transitions - not just for the current discharge, but for future transitions in care.
A deep understanding of care transition models of care will be instrumental to your success in this position. This includes knowledge of discharge planning and post-discharge service provision, knowledge of chronic disease and how to avoid associated hospital admissions and a competency in medication reconciliation.
This experience needs to be supported by an ability to travel to patient’s homes regularly so owing your own vehicle and possessing a full driver’s licence with a good driving record is a must.
