As a Care Navigator, you will provide virtual support to patients participating in the CareComplete suite of programs and will work closely with their team of clinicians in managing patient interactions. Key to the success of this role is your ability to build rapport with people, communicate effectively with clinical staff and adhere to program administration duties.
Your key responsibilities include, but are not limited to:
- Managing regular contact with enrolled patients to review their progress and their participation in the program;
- Fielding patient enquiries and escalating complaints or issues with the relevant stakeholders;
- Redirecting symptomatic patients as appropriate via escalation policies and procedures;
- Working closely with clinical care coordinators to ensure a team care approach is taken with participants across the programs; and
- Managing referrals and or payments for services brokered by the program.
To be successful in this role you will have previous telephonic environment and / or customer service experience. You will have objection handling experience with strong administrative and time management skills. You will have high attention to detail and problem solving skills and advanced computer skills and knowledge of Microsoft programs. You will be available to work between 8 am - 8 pm local time with some flexibility around hours worked.
Health industry experience and allied health assistant background is highly desirable, but is not essential for this position. Importantly, you will be passionate about influencing positive change in healthcare.
